We require the fallowing information displayed in the form below to be able to complete a status certificate request.
You may visit in person our head office with the appropriate payment in the form of certified cheque, money order or cash, providing that we have received the necessary information required for completing the status certificate request.
Once at the head office, we can provide you with copies to be sent to your agent, lawyer or vendor.
